·       Barcode Decals are for Homeonwer(s) / Permanent Residents ONLY!

·       You Must Submit a Clear Picture of VALID Florida Driver Licenses with the Meadow Lakes address as proof of residence for all household drivers of each vehicle.

·       You Must Submit a Clear Picture of each Vehicle Permanent Registration (no temporary registrations) matching the name of a valid resident at the Meadow Lakes address for each barcode decal requested.

·       Barcode Stickers Cost $20.00 Each. Barcodes may not be transferred to other vehicles. New/Replacement vehicles require a new barcode application and fee to cover processing.

·       You may pay by check, made payable to ML at Boca Raton HOA however for faster processing we recommend paying by credit card online for a small convenience fee.

·       Barcodes for motorcycles, commercial vehicles, vehicles with lettering will not be permitted.

·       Allow 7-10 business days for processing – Security will contact you when barcodes are programmed and ready to be placed on vehicle(s).

·       Decals will be installed onto vehicles by GUARDS only.

·       Maximum of 3 barcodes per home without a usable, functioning, empty garage.

·       Up to 5 barcodes per home with usable, functioning, empty double-car garage (Photo evidence required with Request Form).

·       Homeowner must have adequate driveway space to park all bar-coded vehicles clear of the sidewalk and street, between midnight and 6:00am.
This form is only to be used when you have changed the license plate for your vehicle. If you have replaced your vehicle, you must request a new barcode.
Pool Keys Will Only Be Provided to a Home Owner. If you are not the owner of the home (ie. Renter), you must obtain your key from the Homeowner. Your Key can be picked up at the guard house after 7 days of this request.
Event Guest List

Event Guest Lists Must be Submitted 7 days prior to the event to ensure sufficient time for entry into the Gatehouse Computer. Forms submitted less than 7 days prior to the event risk the guard not having access to this list
Mailbox Painting is $25.90 and includes a light wash, dry, and paint of 1 to 2 coats of gloss black paint. It does not include Sanding, Scraping, or ANY Repairs.

The Gazebo, Tables, Grill, Restrooms, and Pool area must be cleaned and free of trash, decorations, and debris from your activity. All trash must be securely bagged in black bags and placed in the trash cans in the fenced area by the parking lot. Failure to completely-clean the gazebo area may result in fining by the Association..*

Lights and ceiling fans must be turned off. Pool furniture, if used, must be re-positioned around the pool. Picnic Tables must be re-positioned to their original location. Grills must be cleaned, if used.*
