Approval is requested to make the following modification(s), alteration(s), or addition(s), described and depicted below, or on additional pages, as necessary. Included are the dimensions, material, color, design, location, and pertinent data.

$750.00 CASHIERS CHECK SECURITY DEPOSIT (REFUNDABLE) REQUIRED FOR PROJECTS LISTED BELOW, TO INSURE SIDEWALKS, SPRINKLERS, CURBING, AND ROADWAYS ARE PROTECTED DURING YOUR MODIFICATION.

1. If permission is sought to spot residential moving container or POD: No semi trailers, ocean containers, or wheeled vehicles of any type may be spotted overnight.
2. Construction dumpsters of any size
3. Driveway “paver” jobs and any other paver jobs requiring pallets of pavers to be forklift delivered.
4. All roof replacement projects.
5. Any project requiring heavy-duty equipment such as a fork lift, front end loader, or backhoe.

ANY DAMAGES TO MEADOW LAKES SIDEWALKS, SPRINKLERS, CURBING, OR ROADWAYS CAUSED BY HOME OWNER AND/OR CONTRACTORS WILL BE REPAIRED AT HOMEOWNER’S EXPENSE AND CHARGED BACK TO HOME OWNER, RESULTING IN FORFEITURE OF DEPOSIT, PLUS ANY ADDITIONAL EXPENSE.

IF YOU ARE PAINTING YOUR HOUSE, DRIVEWAY, FENCE, OR ROOF:

Base Colors are for the body of the house and the garage door (See Garage door note below).

Trim Colors are for the fascia boards, rain gutters, raised areas around windows and doors, and front door (unless optional Front Door Color is selected).

Front Door Colors are optional. They are used for the front door only and require ACB approval.

Garage door should be painted in the house’s Base Color. If homeowner wishes to paint the garage door in the matching Trim Color, ACB approval must be secured.

3. A 2′ by 2′ swatch of each color you are planning must be applied to the house (or driveway/roof/fence) for the ACB to inspect.
4. Do not begin painting until written approval has been received from Crest Management Group.
5. If fence is being refinished, only stain is permitted-NO PAINT. If roof is being refinished-use paint. The driveway/roof/fence color should complement the house color.
6. The ACB committee will not grant approval for any paint colors not listed in the Approved Community Paint Colors binder.

IF YOU ARE PLANNING OTHER IMPROVEMENTS (LANDSCAPING, TREE REMOVAL, FENCE WORK, PATIO, NEW POOL, PAVERS, ENCLOSURE, SHUTTERS, ETC):

1. Homeowner must include a drawing, plans, or other documents describing the changes you wish to make.
2. Homeowner must include a copy of his/her property survey, indicating where improvement will be located.
3. If homeowner is hiring a contractor or other outside labor, furnish insurance and license information.
4. If the improvement includes excavation, homeowner should properly-locate all buried utilities, prior to excavation.

POOL CONDITIONS

1. Contractor must complete the project within 60 days. Beginning date is determined by delivery of any construction materials, removal of fencing, adjusting landscaping, or digging.
2. Construction materials of any type must not remain within view of the street more than 48 hours, including weekends.
3. Failure to abide by these guidelines will result in possible fining imposed against Home Owner.
4. Homeowner acknowledges these conditions by his/her filing of the ACB form for project review and approval.

CONDITIONS GOVERNING THIS APPLICATION

HOMEOWNER HEREBY AGREES TO STIPULATIONS LISTED BELOW:

1. If the modification is not completed, as approved, approval may be revoked and modifications may need to be removed, at homeowner’s expense OR modifications may need to be altered/modified until compliant with ACB application.
2. Homeowner is responsible for any damages done to the common areas, as well as sod, irrigation, landscaping, and drainage on homeowner’s property, as a result of modifications. Homeowner must pay for any expenses incurred to fix damages.
3. Homeowner must comply with all state, county, and/or city building codes, and must obtain all necessary/required permits.
4. Homeowner must abide by decision of the ACB (Architectural Control Board) and Meadow Lakes Board of Directors.
5. If the modification is completed without approval, homeowner may be subject to court action by the Association. Any legal expenses incurred by the Association, regardless of the decision of the court, will be the homeowner’s responsibility.
6. Any expenses incurred by the Association, when reviewing homeowner’s application, for outside professional services/consultation, will be the responsibility of the homeowner.
7. Approved modification(s) must be completed within 45 days (Pool-60 Days) or the approval may be withdrawn. The ACB committee will authorize additional time for completion, under some circumstances.
8. Homeowner is responsible for assuring all contractors, tradesmen, and other hired personnel abide by all HOA rules and regulations. Homeowner agrees to assume full responsibility for any damages attributable to aforementioned personnel.

HOMEOWNER MUST FURNISH LICENSING & INSURANCE INFORMATION FOR ALL WORKERS TO BE CONTRACTED

1. Homeowner must provide proof of insurance for any contractor, or other worker, to be employed for the modifications.
2. Homeowner must provide proof of proper license(s), if required by county and state, for the modifications.
3. Homeowner must provide contractor, and/or other workers, a copy of the approved ACB form and assure contractor and/or workers understand and agree to comply with all HOA rules and regulations.

AN APPROVAL OR DENIAL WILL BE DELIVERED TO HOMEOWNER WITHIN 30 DAYS OF YOUR APPLICATION. A DENIAL CAN BE APPEALED TO THE BOARD OF DIRECTORS AT THE NEXT REGULARLY-SCHEDULED MEETING.
